What is HomeGrown

HomeGrown is a nutrition and physical activity program designed specifically for family’s of young children with Down syndrome. This program is adapted from a nationally recognized early childhood health promotion program to meet your families' unique needs. Our easy-to-use self-assessments, goal setting and action planning tools, and resource library will empower you to take control of your child’s wellness.

Most children in the United States don’t meet nutrition recommendations. Good nutrition is foundational to wellness for all children, and for children with Down syndrome, nutrition is especially important. Optimal nutrition can help prevent chronic diseases later in life and minimize some health complications that children with Down syndrome may be at higher risk of developing.

Physical Activity

Physical Activity

We know that children’s gross motor development is a high priority for family members of children with Down syndrome. In our Physical Activity module, you will learn evidence-based recommendations and practical strategies to help your child meet physical activity guidelines to promote heart health, gross motor development, and overall wellness.

Step 1
Assess
You’ll start by taking a self-assessment in one module. The self-assessment will ask you questions about your family’s current habits in either nutrition or physical activity. This is a no-judgement zone! The self-assessment is simply used to tell you what your strongest habits already are and suggest personalized goals for supporting your child’s wellness.
Step 2
Plan
After taking the self-assessment, you’ll review your personalized goal options and select 3-5 to work on. Then, our website will help you create an action plan to reach the goals that matter to you. Your HomeGrown Coach will be with you to talk through ideas, suggest strategies, and cheer you on!
Step 3
Learn & Act
No matter what goal you set, HomeGrown offers a variety of resources to help you meet your goals. We have videos, handouts, grocery templates, activity cards, and more to help you make your goals a reality.
Step 4
Keep it Up
Every HomeGrown cycle “ends” with re-taking the self-assessment. This is a chance to celebrate your progress and think about what new goals you would like to work on! No one does everything, but everyone can do something! Re-assessing your habits is a chance to keep your healthy momentum going.
